Psoriasis
Psoriasis is a common skin disorder that is characterized by thickened patches of inflamed skin. It occurs in approximately two percent of the population of the US and Europe. Both men and women are equally affected by psoriasis, which usually appears between the ages of 10 and 30.
The disorder results from the abnormally fast production of new skin cells. A healthy skin cell takes 4 weeks to complete the cycle of production, while a psoriasis cell takes less than four days. This abnormal production of cells causes skin cells to accumulate forming thick patches sometimes referred to as plaques. Psoriasis can affect large areas of the skin causing significant physical discomfort. Psoriasis may take different forms, therefore requiring specific treatment for each type. The disease is less common in the summer and can disappear for long periods of time. Psoriasis is considered a long term disorder without a permanent cure. Most attacks of psoriasis can be well controlled with the appropriate treatment.
Symptoms
Psoriasis generally causes the appearance of thick patches of red, inflamed skin that has a silvery scaly surface. It usually affects the elbows, knees, scalp, genitals and buttocks although it can appear in any part of the body. Toes and fingernails which are affected can develop pits and ridges. Itching can be present. Joint tenderness can also accompany psoriasis.
Types of Psoriasis
Discoid or Plaque Psoriasis
Characterized by thick scaly patches appearing on the trunk and limbs. The elbows, knees, and scalp are particularly susceptible. In addition, the nails may become pitted or thickened and can separate from the nail bed.
Guttate Psoriasis
This type occurs most frequently in children and consists of small patches of red, inflamed skin that can develop quickly, especially following infection.
Pustular Psoriasis
Small pustules form in this type of psoriasis and can appear anywhere on the body or can stay confined to the palms or soles of the feet.
Causes
The exact cause of psoriasis is unknown. Although there is a genetic predisposition the the disorder. It results from abnormally fast production of new skin cells with the normal rate of old skin cell removal. As a result, these new live cells accumulate and form the characteristic thick patches, which are covered with dead flaky skin. Attacks of psoriasis have been linked to emotional stress, surgery, physical illness, the faulty utilization of fat, immune dysfunction, alcohol consumption, impaired liver function, incomplete protein digestion, trauma to the skin, poison ivy, sunburn, certain drugs such as lithium and beta blockers, food allergies, and lack of hydrochloric acid.
Conventional Therapies
Doctors will treat new patches of psoriasis early. In mild cases of psoriasis, ultraviolet lamp exposure (phototherapy) may be helpful. Emollient creams are also routinely prescribed. For more severe cases, ointments which contain coal tar and anthralin may be recommended. Corticosteroid drugs, methotrexate, and anti cancer drugs are also used.
Cyclosporine drugs have had some good results although they are not approved by the FDA for treating psoriasis. Tocaltrol is currently being tested for its potential benefits. Freezing smaller areas of psoriasis with liquid nitrogen is also being researched. Skin patches such as Actiderm can more effectively treat affected areas. Some drugs such as Legison can have significant side effects and should be discussed with your doctor. Oxsoralen ultra, a liquid preparation is also routinely used in the treatment of psoriasis. If joint swelling is present, nonsteroidal anti inflammatory drugs may be prescribed. Injecting cortisone directly into an old thick patch of psoriasis can help remove it. In sever cases, intensive ultraviolet therapy may require hospitalization. Continued us of corticosteroid creams can make the skin weak and thin.
